California Community Transitions (CCT) Program at CIL 

The Center for Independent Living (CIL) celebrates and works to make a reality the promise of the U.S. Supreme Court’s landmark Olmstead decision (1999), which states that Americans with disabilities are entitled to receive the supports they need in community-based settings whenever feasible and should not be required to live in institutions in order to receive such supports.  CCT is a federally funded program, administered by California’s Department of Health Care Services, designed to provide residents of long-term care facilities with options counseling about community-based support services and, for residents who would like to return to community living, assistance with doing so.  CIL is a CCT Lead Organization for Alameda County and, as such, assists Medi-Cal recipients who reside in skilled nursing facilities and other long-term care institutions transition into their own homes in the community. 

CIL’s CCT staff can help eligible residents of institutions: 

  • Discover and understand the various living environments and options available to them,
  • Locate and secure affordable housing,
  • Apply for and obtain In-Home Support Services,
  • Receive SSI payments and/or other forms of income directly upon discharge from an institution,
  • Learn/re-learn/improve the Independent Living Skills required to successfully live independently,
  • Pay for the costs of moving (e.g., deposit, home furnishings),
  • Establish relationships with healthcare providers in the community, and
  • Get involved in meaningful and productive activities after moving into their own home

    Once a CCT participant has transitioned out of an institution and into community life, CCT staff will continue working with that participant for at least one year in order to ensure that the participant’s needs are being met. 

    CIL’s CCT staff only serves residents of institutions within Alameda County; however, we can refer interested parties to other CCT programs throughout California. 

    The CCT program is only open to eligible Medi-Cal recipients; however, CIL can help any resident of any institution explore and understand her options and develop a plan for returning to community life if she so wishes.
